Art Intake Forms

  • If your work is accepted for a show, a signed Gallery Release Form AND an Artist Inventory List must be turned in with all art on the designated Art Intake date.
  • If you wish to be paid for the proceeds of your sales, you must have a current year’s W-9 Form on file with us.
