Please follow the Call to Art's submission instructions, and return all required pieces of info in one email to arteast.CWgallery@gmail.com by midnight of the Call's deadline for entries.
A signed Gallery Release form must be turned in with all art on the designated Art Intake date.
You must have a current year's W-9 form on file w/ the bookkeeper if you wish to be paid for the proceeds of your sales.
